Please complete this form by Friday 9th July to GUARANTEE your badges arrive at school before the summer holidays. If you do not submit your figures by then we will send you badges to 25% of your school roll.
Please Note: If your 100% attendance figure varies substantially from those submitted to the authority for the purposes of statutory requirement we reserve the right to charge your school a fee for the extra badges ordered.
